How Do I know If I Need A Reassessment?

Home Care Assistance - How to Talk to Your Loved Ones About Getting Extra Help at Home

If daily life has become more difficult or your current support no longer meets your needs, it could be time to request a reassessment.

You may benefit from a reassessment if:

✔ Your health or mobility has declined

✔ Everyday activities such as showering, dressing or preparing meals are becoming more difficult

✔ You’ve experienced a fall, hospital admission or recent illness

✔ You’re living with increasing memory loss or cognitive changes

✔ Your partner, family member or informal support network is no longer able to provide the same level of assistance

✔ Your current funding isn’t covering the support you now require

✔ You’re receiving CHSP services but finding it difficult to access the help you need

Many people don’t realise they may qualify for additional funding until they speak with someone who understands the system.

Home Care Assistance Central Coast - CHSP Reassessment

Currently Receiving CHSP Services? When Entry-Level Support Is No Longer Enough.

The Commonwealth Home Support Programme (CHSP) is designed for older Australians who require lower levels of assistance to remain living independently at home.

As circumstances change, many people find they need more regular support than CHSP can provide. A reassessment may help you access additional government-funded services including:

  • Personal care
  • Domestic assistance
  • Transport
  • Social support
  • Nursing services
  • Allied health
  • Assistive technology
  • Home modifications
  • Ongoing care management

Home Care Assistance Central Coast - Support at Home Reassessment

Already Receiving Support at Home? Is Your Current Funding Still Right For You?

Support at Home funding is intended to reflect your current care needs—not the needs you had several years ago.

If your health, mobility or personal circumstances have changed since your last assessment, you may be eligible for a higher level of funding.

A reassessment is commonly requested following:

  • Reduced mobility
  • Falls or hospital admissions
  • Progressive medical conditions
  • Increased personal care requirements
  • Memory or cognitive changes
  • Reduced family support
  • Greater clinical or allied health needs

If you’re regularly reaching your funding limit or delaying services because your budget doesn’t stretch far enough, it may be time to explore a reassessment.

Frequently Asked Questions

Our Central Coast team regularly assists older Australians and their families with aged care reassessments. Below are answers to some of the questions we’re asked most often.

Can I request a new aged care assessment if my needs have changed?

Yes. If you’re finding everyday activities more difficult or your circumstances have changed since your last assessment, you can contact My Aged Care to request a reassessment. This helps ensure your funding reflects your current support needs.

What could happen after my reassessment?

Once your reassessment is complete, My Aged Care will review your current situation. If you’re assessed as requiring a higher level of care, you may become eligible for additional Support at Home funding or a broader range of services.

I’m currently receiving CHSP services. Can I still qualify for Support at Home?

Yes. Many older Australians begin with CHSP and later move to Support at Home as their care needs increase. A reassessment will determine whether a higher level of government-funded support is appropriate.

Will I lose my current services while I’m being reassessed?

In most cases, no. Your existing services generally continue while your reassessment is underway, helping to ensure you continue receiving support during the process.

Can Home Care Assistance help before I contact My Aged Care?

Absolutely. Our experienced Central Coast team can discuss your current situation, explain the reassessment process, answer your questions, and help you understand the options available before you begin.
